2008 Swim Lesson REGISTRATION FORM

GENERAL INFORMATION
*Properly fitting swimming attire required for all ages. Thongs, cut-offs and colored t-shirts will not be allowed. Sandals are the only footwear allowed on the pool deck.

*No disposable diapers. Swimmers diapers are available at the facility.

*Patrons under the age of 8 will not be permitted inside aquatic facilities without an adult 18 years or older.

*Children under 48 inches tall are not permitted on the waterslides.
